Possible reasons

Hiccups are involuntary spasms of the breathing muscle. In someone who is seriously ill, they may be linked to reflux, a swollen or irritated stomach, changes in the body’s chemistry, infection, nerve irritation, or a medicine. They can be tiring and distressing, but hiccups alone cannot show exactly how close someone is to death. The person’s overall condition, alertness, breathing, comfort, and ability to eat or drink give the care team more useful information.

A clinician or palliative-care team can look for a treatable cause while keeping the person comfortable. Acid reflux and gastritis are examples of stomach-related problems that may need consideration.

Comfort measures

Tell the treating team when the hiccups began, how often they occur, and whether they interrupt sleep, meals, conversation, or medication. Keep the person upright if that is comfortable, loosen tight clothing, and offer small sips only if they are awake and can swallow safely. Do not force food or fluids. Avoid startling, breath-holding, or giving home remedies that could cause choking. The team may adjust medicines or prescribe treatment aimed at the suspected cause and the person’s comfort.

Contact the hospice, palliative-care, or medical team promptly if hiccups cause distress, vomiting, choking, pain, inability to rest, or difficulty taking essential medicines. Seek urgent help for severe breathing trouble, blue lips, unresponsiveness, or a sudden major change in condition. If the person has an agreed comfort-care plan, use its contact instructions; the team can guide you without requiring a difficult journey.

Ask what cause is most plausible, whether any current medicine may contribute, how to protect swallowing, and which comfort treatment is suitable. Also ask whom to call overnight and what changes should be reported immediately.
