Reading a core antibody result

A positive core antibody result may occur after a current infection or an infection that happened in the past. It is not the same as the antibody produced by vaccination. A negative result may mean there has been no exposure, although timing and the rest of the test panel matter.

Clinicians usually interpret core antibody beside surface antigen, surface antibody and sometimes viral-load testing. This pattern helps distinguish active infection, past infection, immunity from vaccination, or a result needing clarification. Symptoms such as fatigue or dark urine cannot determine what the result means.

What to do with the result

Take the complete report to a clinician and ask for the result pattern rather than relying on the core antibody alone. You may need repeat blood tests or a Liver Function Test. Until the pattern is clear, avoid sharing razors, toothbrushes or needles and tell relevant healthcare professionals.

If you have no evidence of protection, ask whether Hepatitis B Vaccination is suitable. If active infection is possible, a clinician can discuss monitoring, specialist review and measures that protect partners and household contacts.

When to seek care

Seek prompt medical advice if the result follows a recent blood or body-fluid exposure, or if you develop yellow skin or eyes, dark urine, repeated vomiting, abdominal swelling or severe weakness. Urgent help is needed for confusion, unusual bleeding or rapidly worsening illness.
