Hepatitis B first dose: what to expect

The vaccine presents the immune system with a harmless hepatitis B protein so it can prepare a targeted response. It cannot cause hepatitis B infection. Protection develops over time and the first injection does not replace the rest of the recommended schedule. The timing of later doses depends on age, health, vaccine product, and whether vaccination is routine or needed after a possible exposure.

A sore arm, mild fever, headache, or feeling tired can occur for a short time after vaccination. Serious reactions are uncommon, but tell the vaccinator about a previous severe vaccine reaction, a serious allergy, or an illness being treated. Vaccination does not treat an existing hepatitis B infection.

Keep a record of the date and product, and book the next appointment before leaving if possible. If you miss an appointment, contact the clinic rather than restarting on your own; the clinician can update the schedule. Ask whether you need testing because of a past exposure, pregnancy, immune condition, or work involving blood. Until protection is established, use practical measures such as not sharing needles, razors, or toothbrushes.

Seek urgent help for trouble breathing, swelling of the face or throat, widespread hives, fainting, or severe weakness after vaccination. Contact the clinic for a persistent high fever, worsening illness, or a reaction that concerns you. If you may have been exposed to hepatitis B, seek medical advice promptly because vaccination plans after exposure can differ from routine vaccination.
