Hemorrhoids and ulcerative colitis

Hemorrhoids are swollen blood vessels around the anus and commonly cause bright-red blood, itching, soreness, or a lump. Ulcerative colitis is inflammation of the lining of the large bowel and may present with bloody diarrhoea, urgency, mucus, abdominal pain, and fatigue. The presence of hemorrhoids does not explain every episode of bleeding in someone with colitis; the overall pattern matters.

Keep taking ulcerative-colitis treatment as prescribed and record stool frequency, blood, urgency, pain, fever, and weight change. Avoid straining and prolonged sitting on the toilet, and keep the area gently clean. Arrange a GP consultation or contact your colitis team when bleeding is new, increasing, or accompanied by diarrhoea. They can decide whether examination, blood tests, stool tests, or specialist review is needed.

Seek urgent help for heavy or persistent bleeding, severe abdominal pain, a swollen abdomen, repeated vomiting, faintness, confusion, high fever, or inability to keep fluids down. A rapid change in bowel symptoms can signal a significant flare or another problem and should not be attributed to hemorrhoids without assessment.

Ask whether your bleeding pattern fits hemorrhoids, ulcerative-colitis activity, or both. Discuss whether treatment needs adjustment, how to relieve anal discomfort, and whether colorectal cancer screening is due based on your history and local guidance.
