Heavy period but pregnant: what to know

Bleeding in pregnancy can have several explanations, including changes at the cervix, a pregnancy that is not progressing, or a pregnancy developing outside the womb. Bleeding alone cannot distinguish these possibilities. The amount, colour, clots, pain, dizziness, and stage of pregnancy all help a clinician decide what to do next.

If pregnancy has not been confirmed, bleeding may instead be a period or another form of abnormal uterine bleeding. Heavy periods can also occur with uterine fibroids, adenomyosis, endometrial polyps, hormonal changes, or polycystic ovary syndrome. Ongoing blood loss may contribute to iron deficiency anaemia.

Contact a maternity service, gynaecology clinic, or urgent care service promptly if you have a positive pregnancy test and heavy bleeding. Note when it began, how quickly pads become soaked, whether tissue or clots appear, and whether pain is on one side. Do not insert anything into the vagina while bleeding heavily, and take a pregnancy test if pregnancy is possible but unconfirmed.

Seek emergency help for heavy bleeding with severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, weakness, or trouble breathing. These symptoms can signal significant blood loss or a pregnancy complication and should not wait for a routine appointment.

Ask whether you need repeat pregnancy testing, blood tests, or an ultrasound; what bleeding changes should trigger emergency care; and whether your symptoms could be causing anaemia.
