What should you know about heavy breakthrough bleeding pregnancy?

Light spotting can occur when the cervix is more sensitive during pregnancy, including after sex or an examination. Heavier bleeding is different: it may come from the cervix or vagina, but it can also signal miscarriage, an ectopic pregnancy, placental problems, or another condition that needs examination. The amount and colour of blood do not identify the cause by themselves.

A clinician may ask when the bleeding began, how quickly pads are becoming soaked, whether clots or tissue have passed, and whether you have one-sided or severe abdominal pain. They may check your blood pressure and pulse, examine the cervix, and arrange ultrasound or blood tests based on your stage of pregnancy and symptoms.

Use a sanitary pad so you can observe the bleeding; avoid tampons and do not place anything in the vagina until a clinician advises you. Rest while arranging care, keep a note of the bleeding pattern, and take your pregnancy details and medicines with you. Do not drive yourself if you feel faint, weak, or in severe pain.

If you know that your blood group is Rh-negative, tell the maternity team when you call, because bleeding in pregnancy can affect decisions about follow-up treatment. Do not take new medicines, including anti-inflammatory painkillers, without checking that they are suitable in pregnancy.

Seek emergency help now for bleeding that rapidly soaks pads, severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, breathlessness, confusion, or feeling suddenly very unwell. These symptoms can indicate significant blood loss or a pregnancy emergency.

Contact your maternity service promptly even if the bleeding settles, particularly when it is more than spotting, recurs, contains clots or tissue, or occurs with cramps, fever, or unpleasant-smelling discharge. An infection such as Chlamydia can affect reproductive health and may need testing and treatment; it cannot be diagnosed from bleeding alone.

Ask what examinations or tests are appropriate for your stage of pregnancy, whether you need repeat assessment, and which symptoms should prompt an immediate return. You can also ask whether the cervix, an infection, a polyp, or a pregnancy-related cause could explain the bleeding. Endometrial polyps are one possible source of vaginal bleeding outside pregnancy, but an examination is needed to distinguish causes.
