Heat rash or hives: treatment considerations

Heat rash is linked to trapped sweat and often stays in warm, covered, or rubbing areas. Hives can appear suddenly as itchy raised patches that change shape or location. A heat-triggered hive response is possible, but a rash that looks like hives is not automatically heat rash. Photos and timing can help a clinician tell these patterns apart.

For suspected heat rash, cool the skin, remove damp clothing, and avoid further sweating and friction. For possible hives, note recent foods, medicines, infections, insect stings, or heat exposure and avoid a suspected trigger until assessed. A pharmacist can advise whether an antihistamine is suitable for hives; do not use someone else's prescription.

Keep skincare simple and fragrance-free. A bland emollient may help dry irritated skin, while topical corticosteroids are not a general treatment for every itchy rash and may be unsuitable when infection or hives is suspected; see Emollient Therapy and Topical Corticosteroids.

Get emergency help for hives with swelling of the lips, tongue, mouth, or throat, breathing difficulty, wheezing, faintness, or repeated vomiting. Seek medical advice for a first episode, widespread or recurring welts, fever, bruised-looking spots, or a rash that does not behave like ordinary hives or heat rash.

Ask whether the lesions are welts or small sweat-related bumps, how long each individual mark lasts, and whether medication is appropriate. Bring a photo taken during a flare and a list of new products, medicines, foods, illnesses, and heat exposures.
