Heartburn in a twin pregnancy: what to know

Pregnancy hormones can relax the muscle that normally helps keep stomach contents down. As the pregnancy grows, pressure in the abdomen can increase, making burning behind the breastbone or a sour taste more noticeable. Carrying twins may make physical pressure and meal-related discomfort more prominent, but heartburn does not indicate how the babies are developing. It can also overlap with nausea, indigestion, or acid reflux, so describe the full pattern to your maternity clinician.

Try smaller meals spread through the day, eat slowly, and remain upright after eating. Avoid lying flat soon after meals and notice drinks or foods that reliably worsen the burning. Ask your obstetric team or pharmacist before taking an antacid or acid-reflux medicine, because pregnancy, twin monitoring, and other medicines affect what is suitable. Do not start, stop, or share medication based on advice from a friend. Keep hydrated, especially if nausea is also present.

Contact your maternity team promptly for severe or persistent upper-abdominal pain, a severe headache, vision changes, sudden swelling, repeated vomiting, inability to keep fluids down, or heartburn that feels unusual and does not settle. Seek emergency help for chest pain with breathlessness, fainting, vomiting blood, or black stools. These symptoms need assessment and should not be attributed to pregnancy heartburn without review.
