Heartburn in the first trimester of pregnancy

Short answer
Heartburn in the first trimester can occur during pregnancy, but persistent symptoms should be discussed with a clinician.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Burning or sour taste after meals or when lying down
- First step
- Smaller meals and staying upright after eating
- Medicine advice
- Check suitability with a pregnancy clinician or pharmacist
Why it can happen
Pregnancy-related hormonal and physical changes can make reflux easier to notice, although heartburn may also have started before pregnancy. Stomach acid can move into the food pipe and create burning behind the breastbone, a sour taste, burping, or discomfort after meals. Gastritis, a hiatus hernia, and peptic ulcer are other possible causes. The symptom does not by itself indicate a problem with the pregnancy, and it cannot identify a particular cause without considering the full pattern.
Steps that may help
Choose smaller meals and avoid lying down soon after eating. Sit upright while eating, loosen clothing that presses on your abdomen, and identify personal triggers such as spicy food, rich meals, coffee, or citrus. Keep fluids comfortable rather than taking large amounts with meals. Before using an over-the-counter remedy, check with your maternity clinician or pharmacist because pregnancy changes which products are suitable. Acid reflux medication may be an option when advised for you.
When to get medical help
Contact your pregnancy care team if burning is frequent, severe, or accompanied by difficulty swallowing, ongoing vomiting, dehydration, black stools, or upper-abdominal pain. Get urgent help for chest pressure with breathlessness, faintness, sweating, or pain that travels to the arm, back, neck, or jaw. These warning signs need assessment rather than self-treatment.
What to ask
Ask which foods or medicines may be contributing, whether your symptoms need testing, and which relief options are compatible with your pregnancy. Tell the clinician how far along you are and list prescribed medicines, supplements, and remedies.
