Headache on top of the head: what to know

Short answer
A headache on top of the head may be linked to tension, migraine, dehydration, heat, sinus or viral symptoms, or another cause, and its associated features determine how urgently it should be assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Commonly useful notes
- Sleep, meals, fluid intake, heat, stress, timing, and accompanying symptoms
- Emergency symptoms
- Sudden severe pain, neurological change, vision loss, or stiff neck with fever
Possible explanations
A band-like pressure over the scalp may follow muscle tension, stress, poor sleep, jaw clenching, or a long period at a desk. Migraine can be felt at the top of the head and may bring nausea, light sensitivity, or a pulsing quality. Congestion and facial pressure may accompany allergic rhinitis, a common cold, or chronic sinusitis.
Headache with fever, body aches, or marked tiredness may occur during COVID-19 or dengue fever. Thirst, dark urine, dizziness, cramps, heavy sweating, or recent heat exposure raise concern for dehydration or heat exhaustion. The location alone cannot distinguish these possibilities.
Things to try
Drink fluids in small, regular amounts, rest in a cool quiet place, and eat if a missed meal may be contributing. Take screen breaks, adjust your posture, and relax your shoulders and jaw. Keep notes about sleep, stress, heat, food, fluid intake, timing, and any associated symptoms.
If appropriate for your health, use a labelled non-prescription pain medicine as directed. Ask a pharmacist to check it when you take other medicines or have a long-term condition. Book a routine assessment when headaches are frequent, persistent, disruptive, or changing.
When not to wait
Go to emergency care for an abrupt, severe headache, or one accompanied by weakness, numbness, confusion, speech difficulty, fainting, seizure, or vision loss. Seek urgent help after head injury, or with fever and a stiff neck, repeated vomiting, or unusual drowsiness.
Contact a clinician the same day for worsening pain with fever, rash, severe light sensitivity, or significant heat or fluid-loss symptoms. If the headache settles but repeatedly returns, arrange review to identify triggers and a prevention plan.
Useful questions
Ask what features distinguish migraine from tension or illness-related headache, whether your medicines or daily routine contribute, how to use pain relief safely, and when a change in symptoms needs urgent review.
