Possible explanations

One-sided neck lumps commonly involve a lymph node, but skin, muscle, salivary tissue, thyroid tissue, and cysts can also be involved. A recent throat, ear, dental, or skin infection may cause a reactive node, sometimes with tenderness. A firm, painless, fixed, or enlarging lump needs review because lymphoma, head and neck cancer, or thyroid cancer can involve structures in the neck. If cancer is found in a node without an identified starting site, clinicians may describe it as cancer of unknown primary. The side of the neck does not by itself distinguish between these causes.

What you can do now

Arrange a medical appointment and write down when the lump appeared, whether it is changing, and whether it moves when you swallow or turn your head. Note pain, redness, recent illness, dental work, hoarseness, swallowing changes, fever, night sweats, or weight change. A clinician may examine the lump and inspect the mouth, throat, ears, and thyroid. Depending on the findings, ultrasound, blood tests, or a needle sample may be offered. Do not attempt to puncture or drain the lump.

When care should be prompt

Seek emergency care if swelling causes breathing difficulty, choking, or inability to swallow fluids. Seek prompt review if the lump is rapidly enlarging, hard and fixed, or linked with persistent hoarseness, unexplained weight loss, drenching night sweats, or ongoing fever. A lump that remains after an infection has settled also merits assessment. These warnings guide timing, not the final diagnosis.

Useful questions

Ask what structure the lump appears to involve and whether a nearby infection could account for it. Ask whether an ultrasound or needle sample is needed, what follow-up interval is appropriate, and which changes should bring you back sooner. If a result is unclear, ask what the next test would be.
