Hard cough with blood in phlegm: what to know

Forceful coughing can irritate the nose, throat, or bronchial tubes and leave small red streaks in mucus. Acute bronchitis or a common cold may cause cough and phlegm, while bronchiectasis can lead to repeated phlegm production and bleeding. Asthma may cause cough with wheeze or chest tightness. Acid reflux and allergic rhinitis can keep the throat irritated, although blood should not be assumed to come from reflux or allergy. The amount, colour, recurrence, fever, pain, and breathing pattern help guide assessment.

Arrange medical advice promptly, even if the blood appears only as streaks. Note how much you saw, whether it was mixed through phlegm, and whether it followed nosebleed, vomiting, or a hard coughing spell. Avoid smoking and strenuous activity until assessed. Do not start leftover antibiotics or medicines that affect clotting without professional advice. Bring a list of medicines and mention recent infection, travel, or lung conditions.

Call emergency services for more than a small amount of blood, repeated bleeding, severe breathlessness, chest pain, fainting, confusion, or blue lips. Seek same-day care for fever with worsening illness, fast breathing, new wheeze, or chest pain when breathing. A recurring cough with blood needs prompt clinical evaluation even when you feel otherwise stable.

Ask what examination or chest imaging may be needed, whether your medicines could increase bleeding, and which changes in the phlegm or breathing should trigger emergency help.
