Hand, Foot and Mouth Spots on the Bottom

The buttocks are a recognised site for the rash, alongside the hands, feet and mouth. Spots may be red, raised or blister-like and can be uncomfortable under a nappy or tight clothing. Fever, mouth pain and lesions on the hands or feet support the overall pattern. Bottom spots without those features may instead reflect irritation, Folliculitis, Acne or another rash, so location alone cannot identify the cause.

Change nappies promptly, rinse gently with lukewarm water, and pat rather than rub. Give the skin time to air when practical and use loose, breathable clothing. Do not pop blisters or apply fragranced wipes, strong antiseptics or unfamiliar creams. Wash hands after nappy changes and do not share towels, clothing or bedding. Encourage cool fluids and soft foods if mouth sores reduce drinking or eating; obtain age-appropriate pain-relief advice from a clinician or pharmacist.

Seek prompt care if the child drinks poorly, urinates much less, is unusually drowsy, has breathing trouble, or develops severe headache or neck stiffness. Have the bottom examined if redness spreads, the area becomes hot or very painful, or pus, marked swelling or worsening crusts appear. Medical advice is also appropriate when the rash is unusual, symptoms are worsening, or a young infant has fever.

Ask which cleanser or barrier product is suitable, how often to change nappies, how to manage pain, and when the child can return to childcare.
