Can hand, foot and mouth disease happen without fever?

A child may have good energy and no raised temperature while showing small spots or blisters on the hands, feet, around the mouth, or nappy area. Mouth ulcers can cause drooling or reluctance to eat. A rash without fever is not specific to one illness. Chickenpox, a common cold with unrelated skin irritation, cellulitis, or other conditions can look different or overlap, so location, texture, spread, and the child’s general state matter.

Offer cool drinks and soft foods if the mouth is sore, and keep track of how much the child drinks and urinates. Keep blisters clean and do not scratch or burst them. Wash hands after toileting, nappy changes, or touching the rash, and avoid sharing towels, cups, and utensils. Check the temperature if the child becomes flushed, shivery, sleepy, or less active; do not use fever medicine simply because a rash is present.

Arrange medical review if the rash is spreading quickly, painful, hot, swollen, draining, or not behaving like a mild viral rash. Seek urgent help for breathing difficulty, a seizure, severe drowsiness, confusion, a stiff neck with severe headache, or purple spots that do not fade when pressed. Medical advice is also appropriate when the child drinks poorly or has fewer wet nappies, even without fever.
