Hairline fracture in a metacarpal

Metacarpals are the long bones between the wrist and the fingers. A small crack may cause pain over the back or palm of the hand, swelling, bruising or tenderness when making a fist. Finger movement can remain possible, but gripping, pushing and lifting may hurt. After a fall or blow, the injury may be an acute fracture; repetitive hand loading can also produce a stress fracture. A clinician checks sensation, circulation, finger movement and alignment, then selects imaging to define the injury. Do not judge severity by swelling alone.

Remove rings early if swelling is beginning, unless they are stuck and need professional removal. Rest the hand, keep it raised when practical and use a wrapped cold pack briefly. Avoid punching, climbing, heavy lifting and forceful gripping. Do not tape a finger or use a splint tightly without advice, because pressure can affect circulation. Arrange prompt assessment after a direct blow, fall or persistent pinpoint hand pain. A treatment plan may include a splint, activity changes, pain relief and hand therapy to restore motion safely.

Seek emergency care if a finger points abnormally, the hand is visibly deformed, skin is broken over the injury, or a finger becomes pale, blue, cold, numb or difficult to move. Get same-day advice for rapidly increasing swelling, severe pain or inability to use the hand. New loss of grip or pain that persists after rest also warrants assessment, even if bruising is limited.

Ask which metacarpal is involved, whether the fracture is aligned, and how to position or wear any splint. Clarify lifting limits, finger exercises, driving and work modifications, plus the review date. Ask what change in colour, sensation or alignment should bring you back sooner.
