Hairline fracture in the hip after a fall

A hairline fracture is a small crack in bone. Around the hip, the crack may involve the upper thighbone or another part of the joint area. Pain may be felt in the groin, outer hip or thigh, and standing, turning or taking steps can make it sharper. Bruising is not required. A person may still be able to walk, so the ability to bear weight does not rule out a fracture. An examination and imaging help distinguish a fracture from a bruise or soft-tissue injury. An early X-ray may not show every small crack; a clinician decides whether further imaging is needed.

Stop activities that increase hip pain and avoid testing the leg with repeated walks. Use support from another person or a walking aid only as advised, because the safe amount of weight depends on the suspected injury. Keep the hip in a comfortable position and use a wrapped cold pack briefly for swelling. Do not drive yourself if movement is painful or the leg feels unreliable. Arrange urgent clinical assessment, especially if the fall was recent, pain is deep in the groin, or normal walking has changed. Treatment may involve protected weight-bearing, pain relief, follow-up imaging and rehabilitation.

Seek emergency help if the leg looks shortened or rotated, pain is severe, you cannot stand safely, or the foot becomes cold, pale, numb or weak. A fall can also cause injuries elsewhere, so report back pain, head impact or unusual drowsiness.

Ask which bone and area may be affected, whether you should put weight through the leg, and which scan is appropriate if the first image is reassuring. Clarify safe transfers, bathing and stairs, the pain-relief plan, follow-up timing and signs that mean you should return sooner.
