Groin rash in females: what to know

Redness or soreness in the skin folds can follow sweating, friction, tight clothing, shaving, sanitary products, soaps, or fragranced products. Intertrigo develops where warm, damp skin rubs together. A yeast infection may be more likely when the fold is bright red, moist, tender, or dotted with nearby small spots. Ringworm can form an enlarging, scaly edge. Contact dermatitis may match an area exposed to a product, while psoriasis or atopic dermatitis can affect the groin along with other areas. A new medicine can also cause a drug rash, sometimes beyond the groin.

The rash alone cannot identify its cause. Note whether it is on the outer skin, near the vulva, or inside the vagina; vaginal discharge, odour, sores, or pain with urination point to problems that need a different assessment.

Gently wash the outer area with lukewarm water or a mild, fragrance-free cleanser, then pat dry. Wear loose, breathable underwear and change after sweating. Pause new creams, deodorants, wipes, detergents, and shaving until the skin settles. Do not scratch, douche, or apply steroid or antifungal creams inside the vagina unless a clinician has advised it. A pharmacist or clinician can help choose treatment after examining the rash, especially if it is the first episode or keeps returning.

Arrange medical care if the rash is spreading, persistent, recurrent, very painful, oozing, crusted, blistered, or accompanied by fever. Seek prompt assessment for severe swelling, rapidly worsening redness, open sores, or pain with urination. Pregnancy, diabetes, immune suppression, or recent antibiotic use also lowers the threshold for review. A clinician may inspect the skin and take a swab when the appearance does not clearly identify the cause.
