Groin pain during sex: what to know

Groin pain during sex deserves attention because its cause may involve the vulva, pelvic organs, muscles, urinary system, or an infection. Pain at the opening may reflect irritated skin, dryness, or vulvodynia, while deeper pain can be linked with endometriosis, an ovarian endometrioma, or pelvic inflammatory disease. The timing, depth, side, and associated symptoms help guide assessment.

Pain that starts with touch or penetration is different from pain that builds with deeper movement. Burning, itching, or visible skin changes can occur with lichen sclerosus or genitourinary syndrome of menopause. Cramping that follows a cycle may occur with endometriosis. Fever, unusual discharge, bleeding, or urinary symptoms need a clinical review because infection and inflammation can require treatment. A pain symptom is not proof of any one condition; several problems can feel similar.

Pause activities that trigger pain and record when it occurs, where it is felt, and whether it relates to penetration, position, or your menstrual cycle. Use a gentle, fragrance-free approach to vulval care and avoid douching. If dryness contributes, a suitable lubricant may reduce friction. Arrange an appointment if the pain persists, returns, or affects intimacy; an examination and targeted tests can distinguish skin, pelvic, urinary, and muscular causes.

Seek urgent medical help for sudden severe pelvic or groin pain, fainting, heavy bleeding, fever with worsening pain, repeated vomiting, or a painful pregnancy possibility. Otherwise, book a gynaecology or primary-care assessment for new pain, pain after sex, pain with bleeding, or symptoms that do not settle. Bring a medication list and note any discharge, sores, urinary changes, or cycle-related pattern.
