Gout: joint inflammation due to accumulation

Urate is produced as the body processes purines and is usually filtered by the kidneys. If urate levels stay elevated, needle-like crystals may settle in joint fluid or nearby tissues. Immune cells detect the crystals and release inflammatory signals. This can make a joint suddenly painful, warm, red and swollen. A flare may affect the big toe, ankle, knee or another joint. Gout is one possible explanation for joint inflammation; infection, injury and other forms of arthritis can resemble it.

Protect the joint from unnecessary loading, rest it in a comfortable position and apply a wrapped cool pack briefly. Follow the treatment plan given by a clinician. Some medicines calm inflammation during an attack, while separate urate-lowering treatment helps prevent crystals from building up over time. Keep follow-up appointments so treatment can be adjusted to your symptoms, kidney health and other medicines. Do not change a prescribed dose on your own.

Get urgent care for a painful hot joint accompanied by fever, chills, confusion or rapidly increasing swelling. A first flare also needs medical review because gout cannot be confirmed from appearance alone. Contact a clinician when attacks are recurring, movement is becoming difficult, or treatment is causing new symptoms.
