Generalised lichen planus: what to know

Lichen planus can produce small, flat-topped or slightly raised bumps that may look purple, reddish, brown, or grey on different skin tones. In a generalised pattern, areas such as the wrists, ankles, legs, trunk, or lower back may be involved. Itching can be strong, and scratching may create new irritated areas or leave darker marks after the rash settles.

The condition can also affect the mouth, scalp, nails, or genital skin. A clinician may inspect several sites and sometimes take a small skin sample when the pattern needs confirmation. A widespread itchy rash has several possible causes, so its appearance should not be self-diagnosed.

Use fragrance-free moisturiser and lukewarm water, keep nails short, and try not to scratch. A dermatologist may prescribe topical corticosteroids for selected areas. When many sites are involved or symptoms are severe, oral corticosteroid treatment or another medical approach may be discussed. Follow the prescribed schedule and do not stop systemic medicine suddenly without medical advice. Tell the clinician about mouth sores, nail changes, or scalp symptoms because these alter the assessment.

Arrange a review if the rash is spreading, disrupting sleep, leaving extensive colour change, or affecting the mouth, eyes, genitals, scalp, or nails. Seek prompt help for painful mouth ulcers that limit drinking, blistering, pus, fever, or rapidly worsening tenderness. Urgent assessment is needed for eye pain or vision change.

Ask whether you need confirmation with a biopsy, which areas need treatment first, and how to reduce itch without damaging the skin. Discuss whether oral corticosteroid treatment is appropriate and how follow-up will monitor the mouth, nails, or other involved sites.
