What should you know about fundamentals of musculoskeletal ultrasound?

Short answer
Musculoskeletal ultrasound creates live images of muscles, tendons, ligaments, nerves, and fluid-filled spaces while the area is examined and moved.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Uses
- Live assessment of many superficial soft tissues
- Preparation
- Usually no special preparation is needed
What it means
A clinician places gel on the skin and moves a handheld probe over the painful or suspected area. Sound waves form images on a screen, allowing comparison between sides and observation of structures during movement. Ultrasound can show fluid, tendon thickening or tears, some ligament and nerve changes, and the relationship between tissues close to the skin. It does not use ionising radiation.
The quality of the examination depends on the operator, the equipment, and the question being asked. Deep bones, some internal joint structures, and areas hidden behind bone may need another imaging method. A scan is interpreted alongside your symptoms and physical examination.
What you can do
Wear clothing that allows access to the body region and tell the clinician where symptoms begin, what movement triggers them, and whether they change at rest. Most examinations require no special preparation. You may be asked to move the joint or contract a muscle so the clinician can watch the tissue in action.
Ultrasound can also guide procedures by showing the needle and target in real time. For example, it may help a clinician place a cortisone injection accurately when that treatment is appropriate. The scan itself does not decide whether an injection or another treatment is right for you.
When to seek care
Discuss imaging if pain, swelling, weakness, or restricted movement persists and the diagnosis is uncertain. Seek prompt medical attention for a rapidly enlarging swelling, a hot red joint with fever, sudden loss of strength or sensation, or a significant injury with inability to use the limb. These symptoms need clinical assessment rather than imaging alone.
Questions for your doctor
Ask what structure the scan is intended to assess, whether the examination will include movement, and what ultrasound can or cannot show in your body region. If a procedure is being considered, ask how imaging changes the plan and what other options fit the findings.

Questions people ask
Does musculoskeletal ultrasound hurt?
The probe usually causes little discomfort, although pressure over an already tender area may be uncomfortable.
Can ultrasound replace an MRI?
Not always. Ultrasound is useful for selected tissues and movement, while MRI may be better for deep or complex structures.
