There is no single transfer day that fits every cycle. In a natural or modified natural cycle, ultrasound and hormone monitoring help identify ovulation. In a hormone-prepared cycle, the clinic uses the start of progesterone as the reference point. The embryo’s development stage is then aligned with the number of days the womb lining has been exposed to progesterone.

Your period date alone cannot reliably set the appointment. The clinic may adjust the plan when ovulation occurs later than expected, hormone results are unclear, the lining needs further assessment, or an appointment changes. The date is confirmed by the treating team rather than calculated independently.

Attend scans and blood tests when scheduled, report the first day of bleeding accurately, and do not change progesterone timing without speaking with the clinic. Keep a written record of doses and appointment times. Ask what to do if bleeding begins unexpectedly or you cannot attend monitoring.

On transfer day, bring identification and follow the clinic’s instructions about medicines, meals, arrival time, and transport. Afterward, continue the prescribed plan until the pregnancy test or a clinician tells you otherwise.

Call the fertility clinic if you have heavy bleeding, severe pelvic pain, fever, fainting, breathing problems, or rapidly increasing abdominal swelling. Contact them urgently if you take progesterone at the wrong time or miss it, because the team may need to advise you about the schedule. Seek emergency help for severe symptoms or collapse.

Ask which event sets your transfer date, whether the embryo is at an earlier or later development stage, how the clinic handles a late ovulation, and when the date becomes final. Confirm the exact progesterone start time and pregnancy-test date before leaving the appointment.
