Freckles on the bottom of the feet: what to know

Freckles are flat pigment spots, but the soles have thicker skin and are less often inspected than the face or hands. A dark area may also follow friction, a small injury, inflammation, or bleeding beneath the skin. Some pigmented lesions can resemble freckles, so a sole mark should not be named from a photograph alone.

Look at whether the spot is flat, evenly coloured, and similar to other marks. Note its position, outline, and surface. Footwear pressure can make the area sore, but pain or irritation should not be used to decide whether pigmentation is harmless.

Inspect the soles and spaces around the toes in good light. Photograph the mark from the same angle and record whether its outline or colour changes. Reduce rubbing from tight footwear and do not scrape, burn, or apply acids to the spot. Because treatment on thick sole skin requires individual planning, discuss any cosmetic concern with a dermatologist only after assessment; light-based treatments may not be suitable for every lesion or skin type.

Seek a dermatology review for a new spot with no clear injury, or one that expands, becomes uneven, shows different colours, develops a raised or scaly surface, or bleeds. Prompt attention is also needed for a persistent sore, ulceration, or a lesion that changes beneath a nail or near the toe. A mark that is hard to monitor because of its position should be checked rather than ignored.

Ask whether the spot needs dermoscopy, whether friction or old bleeding could explain its appearance, and what follow-up interval is suitable. If removal is discussed, ask how a sole procedure could affect walking, wound care, and the chance of pigment changes.
