What should you know about foreskin pain during sex?

The word foreskin may refer to the clitoral hood, a fold of skin that protects the clitoris. Pain there can be triggered by direct pressure, vigorous rubbing, reduced lubrication, a small tear, dermatitis, thrush, or another infection. Burning or pain at the entrance may occur with vulvodynia, while deeper pain can be linked with conditions such as endometriosis, pelvic inflammatory disease, or an ovarian endometrioma. Lichen sclerosus can cause fragile, itchy, or changing vulval skin.

The timing helps: pain on touch, pain with penetration, and pain that continues afterward can point to different contributors. Pain during sex is a symptom, not a diagnosis.

Stop any activity that causes pain and allow irritated skin time to settle. Use adequate unscented lubricant, avoid perfumed products, and choose gentler positions with control over depth and pressure. Do not scrub, stretch, or self-treat a visible skin change. A clinician can examine the vulva, discuss sexual and pelvic symptoms, and test for infection when needed. Treatment depends on the cause and may include tailored skin care, pelvic-floor support, or treatment for an identified condition.

Arrange a review if pain returns, interferes with intimacy, or comes with itching, discharge, sores, bleeding, urinary symptoms, or a persistent colour or texture change. Seek urgent help for severe pelvic pain, fever, fainting, heavy bleeding, or rapidly increasing swelling. Pain after sexual contact with a new partner also merits timely sexual-health advice.

Ask which area is tender, whether the skin needs testing, how to protect it during healing, and whether pelvic-floor or endometriosis assessment is appropriate.
