What should you know about follicular cysts of skin and subcutaneous tissue?

A follicular cyst develops from structures associated with a hair follicle and may extend into the tissue immediately below the skin. It can feel round, smooth or firm, with the surface either normal or marked by a tiny opening. Some cysts are stable and painless. Others become red, warm, swollen and tender when the cyst wall or its contents trigger inflammation. A lump cannot be identified reliably from a description alone, so an examination matters when the diagnosis is uncertain, the shape changes or the site is repeatedly irritated.

Keep the area clean, avoid pressure and do not attempt home drainage. Observation may be suitable for a quiet cyst that causes no difficulty. For an inflamed cyst, a clinician may consider an intralesional steroid injection in selected situations. If infected material has collected, abscess drainage may be used. Cyst removal is a separate option intended to remove the cyst structure; your clinician can explain wound care, scarring and whether timing should wait until inflammation settles.

Have a persistent, enlarging or recurrent lump examined. Seek prompt care if it becomes rapidly more painful, hot or red, starts draining, or is accompanied by fever or feeling unwell. A lump near the eye, mouth or airway needs faster attention if swelling affects vision, eating, breathing or swallowing.
