Short answer

For suspected cellulitis, a clinician selects the first antibiotic after assessing how far the skin infection has spread and how unwell you are.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Cellulitis signs
Redness, swelling, warmth and pain are common presentations.
Urgency
Rapid spread or marked illness needs prompt medical assessment.

Understanding the treatment choice

Cellulitis is a bacterial infection in deeper layers of skin. People with cellulitis commonly notice skin redness, swelling, warmth and pain; some also develop fever. The first medicine is not chosen from symptoms alone. A clinician considers the affected site, speed of change, allergies, previous antibiotic exposure, other illnesses and whether tablets are likely to be absorbed and tolerated. A local infection may be treated outside hospital, while extensive infection or significant illness may need urgent assessment and medicine through a vein. Athlete’s foot, wounds and skin cracks can provide an entry point for bacteria.

What to do now

Have new or spreading redness assessed promptly, especially when the area is hot, painful or swollen. Marking the edge of redness with a pen can help show whether it is extending, but it does not replace medical review. Rest and raise an affected limb when practical, keep any wound clean and take prescribed medicine as directed. Do not use someone else’s antibiotic or stop early without advice. People with type 2 diabetes should arrange prompt assessment for a suspected skin infection.

When urgent help is needed

Seek urgent care for rapidly expanding redness, severe pain, fever with shaking, vomiting, confusion, faintness, unusual drowsiness or skin that becomes dusky or blistered. These features can signal a more serious infection, including sepsis. Emergency help is needed for breathing difficulty, collapse or severe facial swelling.

Questions for your doctor

Ask whether the findings fit cellulitis, how soon the antibiotic should start helping, whether a wound or fungal problem needs treatment, and which changes require immediate review. Mention allergies, pregnancy, diabetes, kidney problems and all medicines you take.

Questions people ask

Does skin redness always mean cellulitis?

No. Redness can have several causes, so an examination is needed before choosing an antibiotic.

Why might hospital treatment be needed?

Extensive infection, inability to take tablets or serious illness may require closer monitoring and medicine through a vein.
