What should you know about a firm lump on the back of the neck?

The lump may be an enlarged lymph node responding to infection, a skin or soft-tissue growth, or another local change. A neck lump can also be associated with conditions involving the thyroid, lymphatic system, or head and neck. A firm texture alone cannot identify the cause, and many lumps are not cancerous.

A clinician will consider whether it is tender, mobile or fixed, changing in size, and close to the skin or deeper tissues. They may examine the mouth, throat, scalp, thyroid, and nearby lymph-node areas, then arrange imaging or needle sampling if needed.

Arrange a medical assessment if the lump does not settle, grows, returns, or has no clear explanation. Note when you first noticed it, any recent infection or skin problem, changes in size, and symptoms such as fever, night sweating, weight loss, swallowing difficulty, or voice change.

Do not repeatedly squeeze or massage the area. Bring a list of medicines and previous investigations. The clinician may recommend observation for a defined period, imaging, blood tests, or a biopsy depending on the examination.

Seek urgent medical help if the lump is accompanied by difficulty breathing or swallowing, rapidly increasing swelling, severe pain, marked redness, or a new neurological problem. Contact a clinician promptly for persistent growth, unexplained fever, drenching night sweats, or unexplained weight loss.
