Fever, hot head and cold hands and feet: what to know

During a fever, the body adjusts blood flow and temperature regulation. The skin of the face and head may feel hot, while the hands and feet feel cool during chills or reduced peripheral blood flow. Cold hands and feet can also occur without fever in anaemia, iron deficiency anaemia, hypothyroidism or peripheral arterial disease. Those conditions do not explain every acute fever, so the timing matters. Check temperature with a thermometer rather than judging fever by touch alone, and consider the person’s alertness, breathing, skin colour and ability to drink.

Keep the room comfortably cool, remove excess layers and offer regular fluids. Do not use ice baths or vigorous rubbing to warm cold hands and feet. Measure and record the temperature, pulse if you know how, fluid intake and changes in colour or sensation. Use age-appropriate fever relief only as directed on the label, and ask a pharmacist about safe choices for children, pregnancy or long-term conditions. Arrange a routine review if cool extremities recur after recovery or come with fatigue, breathlessness, numbness or pain.

Seek emergency help for confusion, collapse, difficulty breathing, blue or mottled skin, severe drowsiness, a seizure, very little urine, or cold clammy skin with worsening illness. Prompt medical advice is needed for a rapidly rising fever, persistent vomiting, severe pain, or a child who is difficult to wake. Do not wait for hands and feet to warm if other danger signs are present.

Ask whether the temperature and circulation need checking and whether blood tests could assess anaemia or thyroid function. Mention recurrent cold extremities, colour change, walking pain, numbness, fatigue, recent illness and medicines. Ask whether iron supplementation is appropriate; do not start it without guidance.
