Fever that comes back every four hours

Fever-reducing medicine can lower temperature for a limited period without removing the illness causing it. As its effect fades, the temperature may rise again. Viral infections are common causes, but bacterial infections and inflammatory conditions can also produce recurring fever. The timing alone cannot identify the cause.

Check how the temperature was measured, the person’s age, other symptoms, fluid intake and alertness. Cough, breathing symptoms, pain when urinating, abdominal pain, rash or a local painful area can help direct an assessment. Keep a written record rather than dosing in response to every reading.

Offer regular fluids and rest, use light clothing, and avoid cold baths or alcohol rubs. Follow the medicine label exactly, including the minimum interval and daily limit; never combine products with the same active ingredient. Use the correct child formulation and dose, and ask a pharmacist when unsure. Recheck the person’s comfort and alertness as well as the number on the thermometer. Do not give aspirin to children unless a clinician specifically advises it.

Seek emergency care for difficulty breathing, blue or grey lips, a seizure, severe drowsiness, confusion, a stiff neck, or a rash that does not fade when pressed. Obtain prompt medical advice if fever keeps returning, the person is becoming weaker, cannot drink, is passing very little urine, or has significant pain. A young infant with fever needs urgent clinical guidance.
