Fever, chills, body aches, headache and fatigue without cough

The absence of cough makes a primarily airway illness less obvious, but it does not rule one out. Influenza and COVID-19 can present in varied ways. Dengue fever may cause fever, headache, and marked aches, while malaria can cause fever with chills after exposure in a malaria-risk area. Brucellosis is another consideration after contact with infected animals or unpasteurised dairy. Fibromyalgia, hypothyroidism, and Long COVID may contribute to aches or fatigue, but a new fever needs separate assessment rather than being attributed to them automatically.

Rest and take frequent fluids; note your temperature, urine output, pain locations, rash, vomiting, and any change in alertness. Avoid strenuous activity and alcohol while unwell. Tell a clinician about travel, mosquito exposure, animal contact, food and water exposures, sick contacts, pregnancy, and medicines. Do not start antibiotics or malaria treatment without medical advice, because treatment depends on the cause.

Seek urgent care for confusion, fainting, difficulty breathing, a stiff neck, severe headache, seizures, bleeding, severe abdominal pain, repeated vomiting, or very little urine. Fever after travel to a malaria-risk area needs prompt medical advice, even without cough. Arrange assessment if symptoms intensify, do not begin to settle, or you cannot drink adequately.
