Cold sores often begin with tingling, burning, or tightness, followed by grouped fluid-filled blisters on or near the lip. The blisters can break, crust, and heal. A similar-looking mouth lesion may have another cause, so a clinician may need to examine it, especially if it is the first episode or the pattern is unusual.

The virus can spread through contact with a sore, saliva, or items that touch the mouth. It is most sensible to avoid kissing, oral sex, and sharing cups, cutlery, towels, lip products, or toothbrushes while a lesion is active.
