What the pattern may mean

Breathing may feel harder when you lie flat, when a blocked nose or wheeze becomes noticeable, or when an episode of worry changes your breathing rhythm. Night-time breathlessness can occur with asthma, COPD, bronchiectasis, acute bronchitis, anaemia, angina, atrial fibrillation, or a severe allergic reaction. These conditions do not all behave alike: wheeze and cough point toward the airways, while palpitations, chest pressure, ankle swelling, or unusual tiredness add different clues.

Notice whether the feeling starts before lying down, wakes you after sleep, improves when you sit upright, or follows exposure to food, medicine, dust, or an animal. A clinician may ask about these details and listen to your chest.

Sit upright, loosen tight clothing, and take calm, steady breaths. Avoid lying flat if that worsens the feeling. Use a prescribed reliever inhaler exactly as directed, but do not borrow someone else’s medicine. Record the time, trigger, position, cough, wheeze, chest discomfort, and palpitations. Arrange a medical review for repeated episodes, breathlessness with ordinary activity, or symptoms that have changed.

When to seek urgent help

Seek emergency help for severe or rapidly worsening breathlessness, blue or grey lips, fainting, confusion, inability to speak comfortably, sudden facial or throat swelling, or breathing difficulty with chest pressure. Do not drive yourself if you feel faint or severely unwell.
