What should you know about a facial skin rash around the nose and mouth?

Dry, itchy, inflamed skin may occur with atopic dermatitis or contact dermatitis, especially after a new cleanser, cosmetic, mask, toothpaste, or fragrance. A rash that follows a new medicine needs prompt review because a drug rash can sometimes progress. Heat, moisture, and friction in skin folds can contribute to heat rash or intertrigo. Scaly plaques may occur with psoriasis, while an enlarging ring-like patch can suggest ringworm. Intense itch, especially at night or among close contacts, can occur with scabies.

These clues do not identify the cause by themselves. Around the mouth and nose, avoid assuming that every rash is an allergy or infection; a clinician may need to examine the border, scale, cracks, and any involvement of the eyes or lips.

Pause newly introduced or fragranced products and rinse gently with lukewarm water. Pat dry, avoid scratching, and use a bland moisturiser if it does not sting. Do not share towels, cosmetics, or face cloths while an infectious cause is possible. Do not apply steroid, antifungal, or antibiotic creams around the face unless a clinician has advised the specific product and duration.

Record the start date, spread, itch or pain, recent medicines, products, illness, heat exposure, and contact with anyone who has a similar rash. A clear photo can help show how the rash changes between visits.

Seek urgent help for swelling of the lips, tongue, or throat; breathing difficulty; faintness; rapidly spreading hives; blistering; skin peeling; or eye involvement. Arrange prompt assessment for fever, pus, severe pain, a rapidly spreading rash, or a rash beginning after a new medicine. Children, pregnant people, and anyone with reduced immunity should seek advice sooner when the rash is spreading or unexplained.

Ask what features point toward irritation, eczema, infection, or a medicine reaction; whether testing or a skin swab is needed; which products to stop; and how to protect household contacts. Tell the clinician about every new medicine, supplement, cosmetic, toothpaste, and topical treatment.
