What severe shin splints can mean

Shin splints, also called medial tibial stress syndrome, commonly cause aching or tenderness along the inner border of the shin after repeated impact. A sudden change in distance, speed, hills, or training surface can exceed the leg's ability to adapt. Flat feet or a foot that rolls inward may change how force travels through the lower leg. Severe, focal, one-sided pain, swelling, pain at rest, or pain that keeps worsening is less typical and may need evaluation for another injury such as a stress fracture.

How to respond

Pause running and jumping, then use comfortable low-impact movement only if it does not provoke symptoms. Ice wrapped in cloth may soothe the area after activity. Return to impact gradually only when normal walking is comfortable and tenderness has settled. Physiotherapy can assess strength, ankle movement, and training load. Gait retraining or custom orthotics may be considered when movement or foot mechanics continue to overload the shin; these are not suitable for everyone.

When to seek urgent care

Arrange prompt review for pain that is sharply localised, present at rest or night, or associated with swelling. Seek urgent care after trauma, when you cannot bear weight, or if numbness, weakness, unusual tightness, skin colour change, or rapidly increasing pain develops.
