Extranodal non-Hodgkin lymphoma: what to know

Lymphoma is a cancer of lymphocytes, immune-system cells found in lymphatic tissue and other parts of the body. In extranodal disease, lymphoma cells are found in a site outside the lymph nodes, such as the stomach, bowel, skin, brain, bone, or another organ. The word describes the location; it does not identify one single disease.

Doctors need the biopsy result to identify whether the lymphoma is a B-cell or T-cell type and whether it behaves more slowly or aggressively. The affected site can shape the symptoms. For example, disease in the digestive tract may cause abdominal discomfort or altered bowel habits, while disease in the skin may cause a persistent lump, patch, or swelling. Night sweats can occur with lymphoma, but they can also have other causes.

An oncology team usually combines your history and examination with blood tests, imaging, and a tissue biopsy when feasible. A PET-CT scan may help show where active lymphoma is located and support staging. The pathology report guides the choice of treatment; scans alone cannot establish the subtype.

Treatment may involve chemotherapy, immunotherapy, radiotherapy, or a combination. The plan is tailored to the lymphoma subtype, its location, its extent, your general health, and whether the disease has responded to earlier treatment. For selected situations, the team may discuss CAR T-cell therapy or an autologous stem cell transplant. Keep a record of new symptoms, medicines, fever, weight changes, and questions before appointments.

Contact your oncology team promptly for a new or worsening symptom, especially fever, shaking chills, drenching night sweats, unexplained weight loss, rapidly enlarging swelling, persistent vomiting, bleeding, or increasing pain. Seek urgent help for trouble breathing, sudden confusion, weakness, severe dehydration, or an inability to keep fluids down. The urgency depends on the symptom and your treatment, not on the fact that a symptom is associated with lymphoma.
