What should you know about extracapsular extension breast cancer?

Lymph nodes have an outer covering called a capsule. When tumour cells extend beyond that covering into nearby tissue, the pathology report may call it extracapsular extension or extranodal extension. It is a microscopic finding, not a symptom that can be diagnosed from a breast lump alone. The report may also describe the number of involved nodes, the size of deposits, margins and tumour features. Doctors interpret these details together with examination and imaging rather than treating one phrase in isolation.

Ask for a copy of the pathology report and request an explanation of which node is involved and what surrounding tissue was affected. Your team may review breast imaging, biopsy findings and sentinel lymph node biopsy results, then discuss whether more surgery, radiotherapy or medicines are appropriate. A breast cancer gene expression test or genomic tumour profiling may be considered in selected situations; these tests answer different questions and are not suitable for everyone. Take time to discuss benefits, side effects and alternatives.

Contact your breast team promptly for a new or enlarging breast or underarm swelling, worsening wound redness, drainage, fever, arm swelling or increasing pain after a procedure. Seek urgent care for severe breathlessness, sudden weakness, confusion or a seizure. These symptoms need assessment and do not by themselves show that extracapsular extension has spread to the brain.
