What should you know about external cephalic version success?

ECV is an attempt to guide a breech baby into a head-down position using pressure on the abdomen. The clinician considers the baby’s position, whether the baby is engaged in the pelvis, the amount of amniotic fluid, the position of the placenta, the shape and tone of the womb, and whether you have had a previous birth or uterine surgery. These factors can make movement easier or more difficult, but they cannot predict an individual outcome with certainty. A baby can also change position after a turn.

Ask your maternity team to explain how your personal findings affect the likelihood of a turn and whether another attempt would be considered. Discuss what monitoring is available and what you would prefer if the baby remains breech. A successful turn may change the birth plan, while an unsuccessful attempt may lead to a planned Caesarean Section or another option recommended for your pregnancy. Consider practical arrangements for either outcome.

After ECV, contact maternity care urgently for vaginal bleeding, fluid leakage, painful contractions, severe abdominal pain, or reduced fetal movement. These warning signs need assessment independently of whether the baby turned. Report concerns before the planned appointment as well, since the team may need to reassess your pregnancy.

Ask which factors in your scan and history may help or hinder turning, how the baby’s position will be confirmed, what happens if the baby turns back, and how a breech birth plan would be arranged if ECV does not work.
