What should you know about exertional headache aneurysm?

Short answer
An exertional headache can be harmless, but a sudden severe or unusual headache needs urgent assessment to exclude a dangerous cause.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Trigger
- Strenuous activity can bring on headache during or soon after effort.
- Key distinction
- A rapidly peaking, unusual headache is more concerning than a familiar pattern.
- Until reviewed
- Stop the trigger and record onset, associated symptoms, and recovery.
What this headache can mean
An exertional headache starts during or after strenuous activity such as running, lifting, or intense sport. It may be linked to exertion itself, heat, dehydration, muscle tension, or a respiratory illness. Related conditions include [Dehydration](/conditions/dehydration/), [Heat Exhaustion](/conditions/heat-exhaustion/), [Common Cold](/conditions/common-cold/), and [Chronic Sinusitis](/conditions/chronic-sinusitis/).
Rarely, a sudden explosive headache can signal bleeding or another problem involving blood vessels in the brain, including a ruptured aneurysm. The headache alone cannot identify an aneurysm. A clinician will consider its speed of onset, severity, recurrence, neurological symptoms, neck stiffness, illness, and injury.
What to do after an exertional headache
Stop the activity, rest in a cool quiet place, and avoid repeating the trigger until the pattern has been assessed if the headache is new, severe, or recurring. Drink fluids if you can do so safely, especially after sweating or exercise. Note the activity, onset speed, pain location, associated symptoms, and recovery time.
Arrange prompt medical review for a first headache brought on by exertion or a pattern that is changing. Do not rely on pain relief to dismiss a warning sign. Fever, congestion, or recent viral illness may point to [COVID-19](/conditions/covid-19/) or another cause, but testing and examination may still be needed.
When to seek emergency care
Seek emergency help immediately for a thunderclap headache, a headache that peaks very quickly, or pain with weakness, numbness, trouble speaking, confusion, seizure, fainting, vision loss, repeated vomiting, fever, or a stiff neck. Do not drive yourself when neurological symptoms are present. [Dengue Fever](/conditions/dengue-fever/) and [Heat Exhaustion](/conditions/heat-exhaustion/) can also require urgent review in the right setting.
Questions for your doctor
Ask whether the onset and triggers suggest a primary exertional headache, whether urgent brain imaging is needed, what activity to avoid, and which symptoms should send you straight to emergency care.

Questions people ask

No. Headache has many possible causes, but sudden severe or unusual onset needs urgent assessment.
Can dehydration cause a headache after exercise?
Yes, fluid loss can contribute, especially with heat and sweating, but it does not explain every severe headache.
