Why they happen

A cramp can develop when a muscle is repeatedly loaded, fatigued, or used in heat. Sweating and dehydration may add to the problem, while a sudden increase in training or poor conditioning can leave a muscle vulnerable. Muscle strain can cause pain that lasts beyond a brief cramp. Recurrent cramps may also need review for medicine effects or conditions such as peripheral arterial disease, hypothyroidism, chronic kidney disease, vitamin D deficiency, or reduced muscle mass. The cramp itself does not identify one cause.

What to do during a cramp

Stop the activity and place the affected limb in a comfortable position. Gently stretch the muscle and, if possible, lightly massage it; avoid forceful bouncing or returning to exercise while it remains tight. Move to a cooler place after heat exposure and drink fluids gradually. Once the muscle relaxes, resume activity slowly only if movement is comfortable. A warm shower may soothe tightness, while a cool pack can help soreness afterward.

When to seek care

Seek urgent help for cramps with collapse, confusion, severe weakness, chest symptoms, or signs of heat illness. Arrange medical review if pain remains intense, the limb is swollen or discoloured, you have numbness or weakness, cramps repeatedly interrupt activity, or they occur without exertion. Review is also sensible when cramps began after a new medicine or occur with reduced urine, marked thirst, or ongoing fatigue.
