What should you know about excruciating ovulation pain?

Ovulation can cause one-sided lower abdominal discomfort when an ovarian follicle stretches or releases an egg. That discomfort is generally short-lived. Severe pain can have other explanations, including an ovarian cyst, endometriosis, infection, or a problem unrelated to ovulation. An ovarian cyst can sometimes bleed, rupture, or twist the ovary, which needs prompt medical attention.

Note the side, start time, intensity, relation to your cycle, and any urinary, bowel, bleeding, or pregnancy-related symptoms. These details help a clinician distinguish cycle-related pain from an urgent pelvic problem.

Stop strenuous activity and rest in a position that feels comfortable. A warm pack over the lower abdomen may ease cramping. If you can take them safely, use an over-the-counter pain medicine according to its label; avoid self-treating if you may be pregnant or have been told to avoid that medicine. Arrange a clinical review when the pain is severe, recurs, or disrupts normal activities. A clinician may recommend an examination and pelvic imaging.

Seek urgent care for sudden intense one-sided pain, fainting, marked dizziness, heavy vaginal bleeding, fever, repeated vomiting, a swollen or rigid abdomen, or pain with a positive pregnancy test or possible pregnancy.
