What should you know about excision tattoo removal?

After numbing the area, a clinician removes the tattooed section and brings the wound edges together with stitches. The method removes the ink-containing skin in one procedure, but the trade-off is a surgical scar that may be more noticeable than the original tattoo.

The size, location, skin tension, ink depth, and tattoo design affect whether closure is practical. Larger tattoos may not fit into one excision and can require staged procedures; many people are instead assessed for laser treatment, which breaks pigment into smaller particles for the body to clear.

During consultation, discuss the likely scar, alternatives, wound size, activity restrictions, and how stitches will be managed. Keep the dressing and incision clean and dry as instructed, avoid stretching or picking at the area, and protect healing skin from strong sun exposure.

Attend the planned wound review and report any change in the incision. If laser treatment is being considered, ask whether a picosecond laser or Q-switched laser suits the ink colours and tattoo characteristics.

Seek advice for increasing redness, warmth, swelling, drainage, wound separation, fever, or pain that becomes stronger after initially improving. Apply pressure and contact the clinic if bleeding continues through the dressing.

Urgent care is appropriate for severe allergic symptoms such as breathing difficulty, faintness, or swelling of the lips and tongue. A thick, raised, itchy, or expanding scar also deserves review because scar treatment may be possible.

Ask whether excision is suitable for the tattoo's size and position, where the scar will sit, whether staged surgery is needed, which laser alternatives exist, and how long the wound needs protection.
