Understanding the pattern

With erythema nodosum, individual lumps can improve while others appear nearby, creating a migrating pattern. The skin may feel sore and warm, and joint aches or tiredness can occur. This pattern does not identify the cause by itself. A clinician may examine each area and ask about recent infections, digestive symptoms, joint inflammation, medicines and travel or exposure history. Other rashes can move or spread for different reasons, so avoid diagnosing from photographs alone.

Record where each lump begins, how long it remains tender and any symptoms that arrive with it. Rest, leg elevation and cool packs can make movement more comfortable. Wear soft, non-tight clothing and protect sore areas from pressure. Do not apply strong creams or take leftover steroids without advice. If a cause is found, treating that problem may help the skin settle. A doctor may consider oral corticosteroid treatment in selected situations after checking for infection and other reasons it may be unsuitable.

Book a review when nodules keep appearing in new places, return after improving, or come with fever, persistent cough, diarrhoea, eye pain or swollen joints. Seek urgent care for difficulty breathing, collapse, a rapidly expanding hot area, pus, or severe one-sided leg swelling. These features need prompt examination rather than watchful waiting.
