What should you know about epithelioid melanoma?

Epithelioid describes cells that look broad or rounded under the microscope. It is a pathological description, not a conclusion that can be made reliably from a photograph or the surface appearance of a mole. A biopsy report may also describe tumour thickness, ulceration, margins and other features that guide staging. Melanoma can begin in an existing mole or on previously clear skin, so a changing lesion deserves assessment even when it does not look like a classic dark mole.

Arrange prompt assessment with a dermatologist for a changing, bleeding, crusting, itching or asymmetrical lesion. Diagnosis usually involves removing or sampling the lesion for laboratory examination. If melanoma is confirmed, the team may discuss wider surgical excision and, when indicated by the pathology and stage, sentinel lymph node biopsy. Further treatment can include skin cancer surgery, tumour removal surgery or immunotherapy. Keep the pathology report and ask which specialist coordinates the plan.

Seek prompt dermatology review for a lesion that changes in size, shape or colour, develops a new lump, repeatedly bleeds, or fails to heal. Seek urgent medical help for severe headache, new weakness, confusion, seizures or a sudden change in vision, particularly when melanoma has already been diagnosed. These symptoms have many possible causes, but need timely assessment.

Ask what the pathology shows, whether the margins are clear, what stage has been assigned, whether lymph-node assessment is appropriate, and which treatment team will manage follow-up. Ask how often the skin and lymph-node areas should be checked.
