Recognising the pattern

Entry pain may feel like burning, cutting, stinging, tightness, or soreness when penetration begins. It can be linked with dryness, irritation, pelvic-floor muscle tightening, skin conditions, vulvodynia, hormonal changes around menopause, or an infection. Pain can also begin after childbirth, surgery, trauma, or a distressing experience, and anticipation of pain may cause muscles to tense further. Endometriosis and an ovarian endometrioma more often cause deeper pelvic pain, but some people notice more than one pain pattern. Pain is real even when the skin looks normal.

Ways to start getting help

Pause penetration when it hurts and explain the exact location, timing, and sensation to a clinician. An assessment may include gentle external examination, discussion of medicines and hormonal changes, and checking for infection or skin changes. Use a plain, fragrance-free lubricant and allow more time for arousal; avoid perfumed washes and harsh products around the vulva. A pelvic-floor physiotherapist, sexual-health clinician, or counsellor may be part of care depending on the cause. Treatment should be collaborative and never require you to push through pain.

When to arrange prompt review

Arrange medical review for new or persistent entry pain, bleeding after sex, sores, skin whitening or cracking, unusual discharge, fever, pelvic pain, or pain after a new sexual exposure. Seek urgent help for severe pelvic pain, heavy bleeding, faintness, or fever with marked illness. These signs need assessment for infection, injury, or another pelvic problem; the symptom alone does not establish a diagnosis.
