What should you know about end stage ovarian cancer and ascites?

Ascites is a build-up of fluid inside the abdomen. It can make the belly feel tight or enlarged and may cause early fullness, reduced appetite, indigestion, nausea, constipation, back discomfort, or breathlessness from upward pressure. Ovarian cancer is one possible cause, but fluid can also relate to liver, heart, kidney, infection, or treatment problems. The team may examine you, arrange imaging such as a pelvic ultrasound, and sometimes test a fluid sample. The amount of fluid can change, so symptoms matter as much as measurements.

Tell the team if clothes, eating, sleep, movement, or breathing are becoming difficult. Care may include medicines for nausea, pain, constipation, or breathlessness, and a procedure to drain fluid when pressure is troublesome. Chemotherapy or another cancer treatment may be discussed if it is likely to help and fits your wishes. Palliative care can support symptom control, practical needs, and family conversations. Ask for small meals, comfortable positioning, and help with daily tasks rather than forcing food when appetite is low.

Seek urgent help for severe breathlessness, chest pain, fainting, sudden confusion, repeated vomiting, inability to pass stool or urine, a very painful or hard abdomen, fever, or rapidly worsening swelling. Contact the oncology team if drainage-site redness, leakage, increasing pain, or new difficulty eating develops. These changes can signal infection, blockage, or pressure that needs treatment.
