What should you know about emergency fasciotomy?

Muscles, nerves, and blood vessels sit within groups enclosed by firm tissue called fascia. Swelling or bleeding in that limited space can raise pressure and reduce blood flow. This is compartment syndrome, which can follow a fracture, crush injury, severe bruising, or another major limb injury. Symptoms may include escalating pain, pain when a muscle is gently stretched, tight swelling, altered sensation, or weakness.

During fasciotomy, the surgeon makes incisions in the fascia so the compartment can expand. The wound may need careful follow-up, delayed closure, or another procedure while swelling decreases. Treatment decisions are made urgently from examination and the injury pattern.

Suspected compartment syndrome requires emergency hospital assessment; do not wait for home treatment to see if severe, increasing limb pain improves. Tell the emergency team when the pain began, how the injury occurred, and whether numbness, weakness, or increasing tightness has appeared. Keep the injured limb supported as instructed and do not eat or drink if the emergency team advises this before surgery.

After surgery, follow instructions for wound protection, elevation, movement, pain relief, and prescribed medicines. Rehabilitation may begin with gentle movement and later progress to restoring strength, walking, and function. Attend wound and surgical reviews because the open incision needs monitoring.

Call emergency services or go to an emergency department immediately after a serious limb injury if pain is severe or rapidly worsening, the limb becomes unusually firm or swollen, or there is new numbness, tingling, weakness, pale or cool skin, or difficulty moving fingers or toes. After fasciotomy, urgent review is also needed for heavy bleeding, fever, spreading redness, worsening drainage, or a sudden change in sensation or movement.

Ask which compartment was released, whether the wound will be closed later, how to protect it, and which movements are currently allowed. Clarify the warning signs of infection or reduced circulation and the expected milestones for rehabilitation.
