Effects of Heart Failure

The heart is still beating, but it may not pump enough blood to meet the body's needs. Pressure can then build behind the heart, while reduced blood flow can leave muscles feeling weak. Effects vary between people and can change gradually or during a flare-up.

Fluid in or around the lungs can cause breathlessness during activity, when lying flat, or at night. A persistent cough or wheeze may accompany it. Fluid retention can make the ankles, legs or abdomen feel swollen or tight, and shoes or clothing may become uncomfortable.

Reduced circulation and the extra effort of pumping can cause fatigue, weakness, dizziness or difficulty concentrating. Some people notice a fast or irregular heartbeat, reduced appetite, or needing to urinate more at night. These symptoms can also have other causes, so assessment is needed rather than self-diagnosis.

Take prescribed medicines as directed and keep review appointments. Do not stop or change a dose because swelling or breathlessness improves; a clinician should guide adjustments. Ask which pain medicines, supplements or salt substitutes are suitable, since some can affect fluid balance, blood pressure or kidney function.

Follow the fluid and salt advice in your personal care plan. Check for increasing ankle swelling, worsening breathlessness, a new cough, or a noticeable change in your usual stamina, and record changes to discuss with your care team. Gentle activity within your agreed limits, rest between tasks, and avoiding smoking can support day-to-day function.

Seek emergency help if severe breathlessness comes on suddenly, you cannot speak comfortably because of breathing difficulty, you have chest pressure or pain, you faint, or your lips or skin look blue or grey. Sit upright while waiting and follow any emergency plan provided by your clinician.

Contact your healthcare team promptly for breathing that is worsening, swelling that is increasing, rapid changes in your usual condition, repeated dizziness, or a new irregular or racing heartbeat. Early review may identify fluid build-up, an infection, a rhythm problem or a medicine issue.
