What should you know about the effect of goitre?

Some goitres do not change thyroid hormone production. Others occur with an overactive or underactive thyroid. An overactive thyroid may bring a fast or irregular heartbeat, heat intolerance, sweating, shakiness, anxiety, or unexplained weight loss. An underactive thyroid may cause tiredness, feeling cold, dry skin, constipation, or slowed thinking.

Because the thyroid sits at the front of the neck, enlargement can create a visible swelling or a feeling of tightness. A larger swelling may make swallowing uncomfortable, alter the voice, or press on the airway, especially when lying down. The appearance alone cannot show why the gland is enlarged.

Arrange a GP Consultation if you notice a new neck swelling, a change in an existing swelling, or symptoms suggesting altered thyroid activity. The clinician may examine the neck and ask about medicines, family history, and changes in weight, energy, bowel habits, or heartbeat.

A Thyroid Function Test can help show whether the gland is making too much or too little hormone. Further assessment may be suggested if the swelling has nodules or continues to grow. Do not start iodine or thyroid medicine without medical advice, because the appropriate treatment depends on the cause.

Seek urgent medical help if neck swelling causes difficulty breathing, noisy breathing, inability to swallow saliva, or rapidly worsening tightness. Arrange prompt assessment for a persistent hoarse voice, new pain, or a swelling that is enlarging. A goitre is not automatically cancer, but a new or changing neck lump deserves examination.

Ask what may be causing the enlargement, whether your thyroid hormone levels need checking, whether an ultrasound or other assessment is appropriate, and which changes should prompt an earlier review.
