What treatment is trying to change

Eczema reflects a weakened skin barrier alongside inflammation. Treatment aims to restore moisture, calm inflammation and interrupt the itch–scratch cycle. A plan depends on age, body site, severity, skin colour, previous response and whether infection is present. A cream that is appropriate for a thick patch on an elbow may not be suitable for eyelids or skin folds. Improvement in itch can happen before the colour or texture looks normal, so stopping or changing treatment without advice may allow the flare to return.

Making a treatment plan work

Apply a bland, fragrance-free moisturiser generously and often, especially after washing. Use gentle cleansers, lukewarm water and breathable clothing; reduce friction, heat and products that sting. If a clinician prescribes a topical steroid or another anti-inflammatory, follow the stated amount, site and schedule rather than borrowing someone else's medication. Ask how long to continue after symptoms ease and what to do at the first sign of a flare. Keep a note of products and exposures that seem to worsen symptoms. When treatment needs review

Contact a clinician when symptoms remain difficult despite consistent care, return frequently, affect sleep or interfere with school or work. Seek prompt help for increasing pain, warmth, swelling, pus, yellow crusting, blisters or fever because these changes can indicate infection. Get urgent advice for a rapidly worsening rash, facial swelling or breathing difficulty. Do not mask a changing rash with repeated over-the-counter products before it has been assessed.

Ask which treatment is for daily prevention and which is for flares, where each product may be used, and how much to apply. Clarify whether bathing, moisturiser order or dressings should change. Tell the clinician about pregnancy, medicines, allergies, previous infections and products that caused burning.
