Ectopic spotting: what it can mean

Spotting is light vaginal bleeding between expected periods or at another unexpected time. It may occur with hormonal changes, contraception, irritation, infection, cervical dysplasia, polyps, or fibroids. Cervical cancer and womb cancer are less common causes, but unusual bleeding should still be assessed, especially when it keeps returning or happens after sex or after menopause.

If you could be pregnant, spotting has a different level of urgency. Ectopic pregnancy can present with vaginal bleeding, and the symptom does not show where the pregnancy is located. A pregnancy test and clinical assessment are needed rather than trying to identify the cause from bleeding alone.

Note when the spotting began, its colour and amount, whether it follows sex, and whether you have pain, fever, discharge, or a missed period. Arrange a medical review for unexplained or repeated bleeding. Testing may include a pregnancy test, examination, infection testing, or imaging. Chlamydia can cause bleeding and may need testing and treatment for partners. Avoid self-diagnosing from the pattern of spotting.

Get urgent help if you may be pregnant and have pelvic pain, one-sided pain, shoulder-tip pain, fainting, severe dizziness, or bleeding that is becoming heavy. Seek prompt care as well for bleeding with severe pain, fever, or feeling very unwell. These warning signs need assessment based on symptoms, not on how much spotting is typical for you.

Ask whether you need a pregnancy test, infection screening, cervical examination, or ultrasound; what might explain the timing of the bleeding; and which changes should prompt emergency care.
